Welcome to your Pre-Assessment Questionnaire and Step 3 of the Intake Process!
Before we get to the work, it's important to create a plan. Stephan Curry did not grow up to break the NBA Career 3-point record without a master plan and the chickens in Chicken Run did not make it out the coup to freedom without a plan of attack.
Please set aside 15-20 minutes to fill out the assessment and be as detailed as possible. The more detail you provide, the easier it will be to address your specific roadblocks originating form your life experience and health history.
